Abstract Mutations in the Filamin A (FLNA) gene cause a wide spectrum of multisystem disorders, including epilepsy, and carry high reproductive risks for women of childbearing age. Traditional prenatal diagnosis is limited by invasiveness, gestational age constraints, and limited options after adverse findings. Preimplantation genetic testing for monogenic disorders (PGT-M) offers a safe, effective, and non-invasive preconception strategy to reduce such risks. Here, we report a 32-year-old woman with epilepsy caused by a heterozygous FLNA mutation who successfully delivered two healthy infants following PGT-M. We also identified a novel FLNA mutation, expanded the known mutational spectrum of the FLNA gene, and summarized the clinical and genetic characteristics of FLNA-related disorders by literature review. Our study demonstrates the clinical value of PGT-M in preventing birth defects and reducing reproductive risks in women with epilepsy caused by FLNA mutations. We propose that PGT-M can be used as a routine preconception intervention for women with pathogenic FLNA mutations to improve clinical outcomes and provide a reliable clinical strategy for genetic counseling and reproductive management.
