Systemic Inflammatory Markers and Peripheral Lymphocyte Subsets as Predictors of Epilepsy Following Acute Ischemic Stroke: A Retrospective Cohort Study Running title: Biomarkers of Post-Ischemic Stroke Epilepsy

Abstract Objective To explore the relationship between peripheral inflammatory indicators, lymphocyte subsets and the risk of epilepsy after acute ischemic stroke (AIS), and to screen potential predictive markers for post-AIS epilepsy. Methods A single-center retrospective study was conducted on 435 patients with AIS. According to whether epilepsy occurred within 1 year after stroke, patients were divided into epilepsy group (n = 21) and non-epilepsy group (n = 414). General data, inflammatory indicators, lymphocyte subsets, and National Institutes of Health Stroke Scale (NIHSS) score at admission were collected. Spearman correlation and multivariate logistic regression were used to analyze risk factors, and ROC curve was applied to evaluate the predictive value. Results The incidence of post-AIS epilepsy was 4.83%. Compared with the non- epilepsy group, the epilepsy group had higher NLR, IL-6, CRP levels and admission NIHSS score, while lower percentages of CD4 + T cells, CD8 + T cells and NK cells (all P
