Risk Factors, Clinical Features and Long-Term Neurodevelopmental Outcomes of Neonatal Seizures: A Retrospective Analysis Based on Three Chinese Clinical Centers

[Preprint - wstępne wyniki] Badanie wykazało, że u noworodków z padaczką najpoważniejszym zagrożeniem dla przyszłego rozwoju są wrodzone zaburzenia metabolizmu, mutacje genów oraz wiele przyczyn razem. Urodzenie się z niedowagą również pogarsza rokownictwo. Natomiast płeć noworodka, wiek rodziców czy sposób porodu nie wpływają na przebieg choroby i jej wyniki.

Oryginalny abstract (angielski)

Abstract Objective To explore associations between high-risk etiologies of neonatal seizures and long-term neurodevelopmental, epileptic and mortality outcomes, supporting early prognostic evaluation and individualized interventions. Methods This observational cohort enrolled 127 seizure neonates from three NICUs, stratified into nine etiology subgroups. Perinatal, seizure, etiological and therapeutic data were extracted from electronic medical records. Standardized 3-, 6-, and 12-month follow-ups assessed neurodevelopment at 12 mouths of age, unprovoked epilepsy and all-cause death. SPSS 26.0 was used for descriptive statistics, intergroup comparisons and multivariate Logistic regression (α = 0.05). Results Sex, parental age, gestational age and mode of delivery showed no prognostic correlations (P > 0.05). Low birth weight (OR = 0.415, 95%CI: 0.173 ~ 0.995, P = 0.049) independently predicted poor outcomes. Inborn metabolic disorders(β = 3.937, OR = 51.265, 95% CI: 5.27 ~ 498.698, P = 0.001), mixed etiology༈β = 1.229, OR = 3.418, 95% CI: 1.033 ~ 11.314, P = 0.044༉and gene mutations༈β = 1.132, OR = 3.102, 95% CI: 1.078 ~ 8.917, P = 0.036༉were adverse prognostic risk factors. The time of initial seizure onset across nine etiological groups showed no overall statistically significant intergroup difference (H = 7.977, P = 0.436). Conclusions Inborn metabolic disorders, mixed etiology and gene mutations independently increase long-term adverse risks, alongside low birth weight (BW).
