Identification and Functional Characterisation of a Novel ASAH1 Gene Variant in Two Unrelated Indian Individuals with Childhood-Onset Spinal Muscular Atrophy

[Preprint - wstępne wyniki] Naukowcy odkryli nowy wariant genetyczny genu ASAH1 u dwóch pacjentów z Indii z dziecinnym zanikiem mięśni i napięciami padaczkowymi. Wariant ten powoduje obniżenie poziomu enzymów rozdzielających tłuszcze w komórkach pacjentów, jednak mechanizm choroby może różnić się od zaproponowanego wcześniej gromadzenia się ceramidów. Badanie podkreśla, że do prawidłowego zdiagnozowania chorób genetycznych należy łączyć wyniki testów genetycznych, badań enzymatycznych i obserwacji klinicznych.

Abstract Pathogenic variants in the ASAH1 gene cause a clinical spectrum ranging from Farber disease to spinal muscular atrophy (SMA) with progressive myoclonic epilepsy (SMA-PME), possibly depending on residual acid ceramidase activity. We report two unrelated patients from the southern part of India with childhood-onset neuromuscular weakness. Whole-exome sequencing identified a novel homozygous missense variant in both cases, c.127T > A, p.(Tyr43Asn) in the ASAH1 gene (according to NM_177924.5), with parents confirmed as heterozygous carriers on Sanger sequencing. Acid ceramidase levels measured in patient-derived leukocytes or EBV-transformed lymphoblastoid cells were reduced, indicating a possible pathogenic nature of this variant. In-vitro functional assays, however, did not demonstrate any aberrant mRNA splicing. In addition, ectopic expression of the mutant cDNA in cultured human cells led to acid ceramidase activity and intracellular ceramide content similar to those obtained with the wild-type cDNA. This suggests that the primary mechanism underlying the pathogenesis of SMA-PME in the patients reported here may differ from ceramide accumulation. Our study identifies a novel missense ASAH1 variant associated with reduced acid ceramidase activity in patient cells, which highlights phenotypic variability ranging from SMA to SMA-PME, and underscores the importance of combining genetic, enzymatic, biochemical, functional, and clinical evidence in variant interpretation.
