Abstract Impairments in social cognition are prevalent in neurological and psychiatric conditions, impacting social functioning and serving as potential transdiagnostic markers. Despite being recognised for over a decade in the DSM-5, clinical implementation of assessment is limited due to a lack of well-validated, transculturally applicable, and ecologically valid tests. We developed COSIMO, a 5-minute online tool for emotion recognition, using demographically diverse and ecologically valid video stimuli. We evaluated COSIMO in a cross-sectional validation and normative study. In a German-speaking sample (n = 2299) we found diagnostic accuracy with 52–61% sensitivity and 67–84% specificity. Classifier performance was robust for autism spectrum disorder (AUC = 74%) and temporal lobe epilepsy (AUC = 68%) and expectedly lower for ADHD (AUC = 58%). COSIMO showed a low correlation with general cognitive impairment (τ = 0.27, p = .064), confirming discriminant validity, and medium correlations with existing social cognitive tests of related constructs (τ = 0.34–0.50), supporting convergent validity. Reliability (r = 0.45–0.56) reflected the trade-off between test brevity and complexity, without compromising clinical validity. COSIMO offers a time-efficient solution for social cognition screening and is available in multiple languages for research and clinical use.
