Epilepsy, Antiseizure Medications, and Orthodontic Care: A Bibliometric and Thematic Analysis of Craniofacial Development, Oral Health, and Orthodontic Treatment

Oryginalny abstract (angielski)

Abstract Background Epilepsy and long-term antiseizure medication (ASM) exposure are associated with oral, periodontal, and craniofacial effects, yet the specific orthodontic literature in this population has not been mapped systematically. Objective To characterize the size, thematic composition, and chronological trend of the PubMed-indexed literature linking epilepsy/ASMs to orthodontics, craniofacial development, oral health, and maxillofacial trauma, and to identify the subset of studies directly addressing orthodontic care. Methods Two PubMed exports (a broad MeSH-based export and a focused orthodontic/craniofacial Title/Abstract export) were combined and deduplicated by PMID. Records were screened using an explicit topical eligibility rule requiring an epilepsy/seizure/ASM link plus one of five target themes (periodontal/gingival effects, oral health/dental management, craniofacial development/bone biology, dental/maxillofacial trauma, or direct orthodontic care). The resulting 209-record corpus was used for descriptive bibliometric and thematic mapping based on titles and available bibliographic metadata. All direct orthodontic records and a priority subset of craniofacial/maxillofacial records were additionally verified against PubMed records, including abstracts and MeSH terms where available. Results From 1,671 raw records, 341 duplicate PMIDs were removed, leaving 1,330 unique records. Focused eligibility screening produced a mapping corpus of 209 publications spanning 1951–2026. Periodontal/gingival effects were the largest theme (101/209, 48.3%), followed by oral health/dental management (68/209, 32.5%), craniofacial development/bone biology (19/209, 9.1%), and dental/maxillofacial trauma (16/209, 7.7%). Only five publications (2.4%) directly addressed orthodontic care or orthodontic tooth-movement biology, ranging from a 1981 clinical commentary to two 2026 experimental/human-cohort studies linking ASM exposure to altered bone remodeling and orthodontic treatment outcomes. Conclusions Direct orthodontic evidence in the epilepsy/ASM literature is sparse relative to the much larger periodontal and general oral-health literature, despite biologically plausible mechanisms (altered bone remodeling, gingival overgrowth, craniofacial growth effects) that are directly relevant to orthodontic diagnosis and treatment planning. This imbalance represents a defined research gap.
