Validation of the Spanish version of the Epilepsy Anxiety Survey Instrument (EASI) and its brief version (brEASI) in Colombian patients with drug-resistant epilepsy

OBJECTIVES: To evaluate the reliability, construct validity, and screening performance of the Spanish versions of the Epilepsy Anxiety Survey Instrument (EASI) and its brief counterpart (brEASI) in Colombian patients with drug-resistant epilepsy. METHODS: After obtaining authorization from the original authors, the EASI and brEASI underwent forward and backward translation into Spanish. Adult patients (≥18 years) with drug-resistant epilepsy who provided informed consent and were able to understand and complete the questionnaires were included. Participants completed the validated Spanish versions of the Generalized Anxiety Disorder-7 (GAD-7) and Neurological Disorders Depression Inventory for Epilepsy (NDDI-E). Anxiety disorders were assessed using the Mini International Neuropsychiatric Interview (MINI). Receiver operating characteristic (ROC) curve analyses were performed to evaluate screening performance using the GAD-7 screening threshold and MINI-defined anxiety disorders as comparators. RESULTS AND CONCLUSION: A total of 91 patients were enrolled from the epilepsy clinic at Hospital Internacional de Colombia. The EASI and brEASI showed high internal consistency, with Cronbach's alpha values of 0.935 and 0.836, respectively. The prevalence of current MINI-defined anxiety disorders was 65.9% (60/91). ROC analyses showed acceptable-to-good screening performance. When using the GAD-7 screening threshold as the comparator, AUC values were 0.798 for the EASI and 0.804 for the brEASI. When using MINI-defined anxiety disorders as the diagnostic reference standard, AUC values were higher, reaching 0.894 for the EASI and 0.864 for the brEASI. The Spanish EASI and brEASI demonstrated excellent reliability and acceptable-to-good screening performance, supporting their use for systematic identification of anxiety symptoms in patients with drug-resistant epilepsy.
